Title: Evolution and Transformation Vectors of the Compulsory Health Insurance System in the Russian Federation: Institutional Analysis and Prospects for a Risk-Based Model
Objective – to identify the institutional features of the Russian compulsory health insurance (CHI) model, to reveal systemic dysfunctions of the current mechanism, and to substantiate the directions for its adaptation to the criteria of healthcare performance and quality, considering the socio-economic risks of reform.
Methods and methodology – systemic analysis of regulatory legal acts, content analysis of scientific publications and expert reports, comparative analysis, as well as a case study method for examining foreign experience in implementing risk-based health insurance models.
Results – persistent dysfunctions of the current model are identified: the erosion of the individual risk-based nature of insurance; territorial disintegration of tariff policy; exclusion of fixed asset reproduction costs from the tariff base; and the simulated (formal) nature of quality control. Based on the analysis of Russian and foreign cases (the Netherlands, Germany), practical recommendations are developed for introducing elements of a risk-based model into the Russian compulsory health insurance system.
Application area – the results can be used in the development of regulatory legal acts governing the CHI system; in the activities of the Federal and Territorial CHI Funds and health insurance organizations; in the formation of regional tariff agreements; in the process of digital transformation of healthcare; and for minimizing social and economic risks of CHI system reform.
Conclusions – the implementation of the proposed set of measures will transform the CHI system from a passive financing tool for healthcare volumes into an active mechanism for managing quality and efficiency, focused on final treatment outcomes.
