Последствия реформ фактически снизили потенциал отечественной системы здравоохранения, вызвали рост показателей заболеваемости населения, и замечания к эффективности ее существующей модели деятельности выдвигают теперь как органы управления, так и работники отрасли, население, общество в целом. Цель – проведение исследования актуальных проблем и ограничивающих факторов деятельности отечественных медицинских учреждений в системном единстве элементов и их взаимосвязей. Методы исследования включают системно-ситуационный и структурный анализ реализованной политики медицинской деятельности в масштабах страны. Применялись статистические методы изучения состояния отрасли, динамики и направления развития его основных показателей на достаточно длительном промежутке времени в период с 1990 г. по настоящее время. Результаты. Выявлены основные проблемы развития отрасли, показатели роста заболеваемости населения во весь постреформенный период по настоящее время, нарастание негативной тенденции ухудшения финансового и материального обеспечения, снижения качества медпомощи. При приведении к сопоставимым значениям объемных показателей финансового обеспечения обнаружилось, что темп роста доходов и расходов национальной системы здравоохранения значительно снижается, и значительно отстает от уровня инфляции. Проведена систематизация и анализ основных проблем существующей модели здравоохранения, основанной на обязательном медстраховании предприятий и организаций. Переход к исключительно бюджетной основе не сможет значительно изменить сложившееся положение дел, так как не обеспечит значимое увеличение финансирования здравоохранения. Нужна комбинированная и сбалансированная по экономическим интересам и ответственности ее модель на принципах государственно-частного партнерства. Заключение. Реализованная модель здравоохранения накопила в своем применении целый ряд достаточно существенных проблем, тормозящих вывод ее на целевые показатели жизни и здоровья населения и в существующем виде эта модель и на перспективу не сможет обеспечить сценарии поступательного развития, поэтому необходима ее комплексная модернизация.
The consequences of the reforms have actually reduced the potential of the domestic health care system, caused an increase in the incidence of the population, and comments on the effectiveness of its existing model of activity are now put forward by both management bodies and industry workers, the population, and society as a whole. The purpose of the study is to conduct a study of topical problems and limiting factors of the activities of domestic medical institutions in the systemic unity of elements and their interrelations. The research methods include a system-situational and structural analysis of the implemented policy of medical activity on a national scale. Statistical methods were used to study the state of the industry, the dynamics and direction of development of its main indicators over a fairly long period of time in the period from 1990 to the present. Outcomes. The main problems of the development of the industry, the indicators of the growth of the morbidity of the population in the entire post-reform period to the present, the growth of the negative trend of deterioration of financial and material support, and the decline in the quality of medical care are revealed. When the volume indicators of financial support are brought to comparable values, it was found that the growth rate of income and expenditure of the national health care system is significantly decreasing, and significantly lagging behind the inflation rate. The systematization and analysis of the main problems of the existing model of health care based on compulsory medical insurance of enterprises and organizations is carried out. The transition to an exclusively budgetary basis will not significantly change the current state of affairs, since it will not provide a significant increase in health care financing. We need a combined model balanced in terms of economic interests and responsibilities based on the principles of public-private partnership. Conclusion. The implemented model of health care has accumulated a number of quite significant problems in its application, hindering its implementation of the target indicators of life and health of the population, and in its current form this model will not be able to provide scenarios of progressive development in the future, so its comprehensive modernization is necessary
